Appendicitis is a common occurrence in both adult and pediatric populations. The condition most commonly occurs between the ages of 10 and 20 years with a lifetime risk of 8.6% and 6.7% for males and females respectively.

The pooled incidence of appendicitis or appendectomy (in per 100,000 person-years) was 100 (95% CI: 91, 110) in Northern America, and the estimated number of cases in 2015 was 378,614. The pooled incidence ranged from 105 in Eastern Europe to 151 in Western Europe.

Laparoscopic or “minimally invasive” surgery is a specialized technique for performing surgery. In the past, this technique was commonly used for gynecologic surgery and gall bladder surgery.  With the recent development of laparoscopic surgery during the past 30 years, the devices built for the surgery have progressed through technological innovation and the recycling of former technology. CCD camera technology has contributed significantly to the success of the 1st generation of laparoscopic cholecystectomy. The growth of modern laparoscopic surgical devices has been largely driven by needs.

The early slow pace of endoscopic and laparoscopic evolution was in large part related to the limitations of technology. The introduction of the computer chip television camera is a key event in the development of laparoscopy, as the procedure can be conducted while viewing a projected image of the abdominal contents. As it provides a better view of the internal structures, this also allowed free movement of the surgeon’s hands, making it easier to perform more complex procedures. Before the development of the camera, this approach was reserved only for diagnostic purposes and a few simple surgical procedures in gynecologic applications.

Single-port laparoscopic surgery (SPLS) is a rapidly evolving technology in the field of minimally invasive surgery. Development of innovative articulated or bent instruments, adjustments in laparoscopes, and developed special multi-lumen access devices allowed simultaneous multiple instruments insertion facilitated the acceptance of this technique. Increasing application of this technique has been described through case reports and case series proving it is a safe and feasible technique for the surgical treatment of benign and malignant diseases expanded within general, urological, gynecological, and pediatric surgery. Recent publications described using SILS for appendectomy, cholecystectomy, colectomy, adrenalectomy, splenectomy, bariatric surgery, hysterectomy, ovarian cystectomy, and hernias repair. The interest in the technique has gradually progressed toward assessing its efficacy compared with established techniques in minimally invasive surgery. Comparative studies with standard and hand-assisted laparoscopic surgery have shown that SPLS maintains the benefits of minimally invasive surgery.

Lack of expertise in laparoscopic surgeries will hamper the market growth

Laparoscopy is currently considered the gold standard approach for a range of surgical treatments. While these minimally invasive procedures offer significant advantages in inpatient recovery, they also require complex surgical skills that are difficult for novices to learn and highly vulnerable to error. Moreover, research indicates that 80% of major laparoscopic complications occur early on in a surgeon’s career (often within the first 100 cases), and can lead to disastrous patient outcomes including permanent injury or fatality.

The benefits of laparoscopic surgery are clear but when complications occur, they tend to be serious. COVID-19 Impact Analysis

During the first stage of the COVID-19 pandemic, there was a significant increase in the rate of complicated appendicitis.  In the era of the COVID-19 pandemic, there is a concern for a delayed referral of patients and thus a more advanced presentation of the disease leading to a prolonged and complicated course. One hundred twenty-three patients underwent a laparoscopic appendectomy in the study period in 2020, compared with 109 who had surgery in 2019. During the COVID pandemic, 41 patients presented with complicated appendicitis versus 22 patients in 2019. Despite the growing consciousness of the phenomenon, very little attention is focused on the effects of the delay in diagnosis and management of surgical diseases. A recent international web survey reported increased non-operative management of Medical Laparoscopic Robots appendicitis during the COVID-19 outbreak.

The BITRACK system progressed through technical development in 2018 and has begun FDA/CE approval processes with projected availability by 2021.

The hospital segment will dominate the laparoscopic appendectomy market

In most cases, an appendectomy is an emergency surgery and will require a hospital stay. The patient may opt for either an open appendectomy or a laparoscopic appendectomy. This will depend on the condition and healthcare provider’s practices.

Most patients who need a lap appendectomy enter the hospital through the Emergency Department and are taken to surgery within a few hours.  Compared with open appendectomy, laparoscopic appendectomy has the advantages of less postoperative pain, better cosmesis, and shorter hospital stays. Furthermore, laparoscopic surgery probably reduces hospital costs by shortening hospital stays and providing early postoperative recovery.
