Fomepizole (4-methyl pyrazole) is a nonspecific ADH inhibitor currently being used as an antidote for methanol and ethylene glycol poisoning. Fomepizole can treat intoxication caused by other drugs of abuse by inhibiting ADHs and preventing the formation of toxic metabolites. ADHs can be activated by the farnesoid X receptor (FXR) and other transcription factors. In the clinic, drug-drug interactions for ADHs have been observed between ethanol and therapeutic drugs and between fomepizole and ADH substrates.

The activity of these enzymes in tumour tissue is reflected in the serum and may be useful in diagnosing brain neoplasms.

Alcohol dehydrogenase is a polymorphic enzyme in multiple isoenzymes classified into several classes and found in various organs. ADH is involved in the metabolism of many biologically important substances, catalyzing the oxidation or reduction of a diverse range of substrates. The best-studied function of ADH is protection against excess ethanol and other exogenous xenobiotics and lipid peroxidation products. Alcohol dehydrogenase isoenzymes are also involved in the metabolism of retinol and serotonin. Cancer tissues have significantly higher total alcohol dehydrogenase activity than healthy organs (e.g., liver, stomach). Changes in the activity of specific ADH isoenzymes in the sera of patients with various cancers (particularly the digestive system) appear to be caused by the release of these isoenzymes from cancer cells. They may serve as markers of this cancer. The presence of specific ADH isoenzymes in the serum may indicate the location of cancer. Alcohol dehydrogenase may also be useful in the detection of non-cancerous liver diseases (e.g., viral hepatitis, non-alcoholic cirrhosis).  For instance, ADH and ALDH isoenzymes can be secreted from cancer cells and found in patients' sera, suggesting they could be used in cancer diagnostics. ADH isoenzymes are being studied as tumor markers in various malignant diseases.
